List the names of the persons, other than the surviving spouse, you wish to be the decision maker concerning your incapacitation and estate upon your death.
Please provide the names of the Personal Representative & POA below. It is best if you can provide a Primary and Secondary choice.

List the names of the persons, other than the surviving spouse, you wish to make any major medical decisions on your behalf. Please provide the names of the Advanced Health Care Directive/POA below. It is best if you can provide a Primary and Secondary choice.

List the names of the persons that you wish to raise children under the age of 18, if both spouses die. If choosing a couple, please indicate who you wish to gain custody in the case of their divorce. Please provide the name of the persons below. It is best if you can provide a Primary and Secondary choice.

List the names of the persons, other than the surviving spouse, that you wish to act as a successor trustee to the living trust after the death of both of you. The successor trustee will be the person that will carry out the wishes and terms stated in your trust. The successor trustee will also be the executor of your pourover will. Please provide the names of the successor trustee below. It is best if you can provide a Primary and Secondary choice.
